- 4 certified copies of the student’s ID
- Both parents/ Guardian’s Certified ID copies
- Both parents’/ Guardian proof of income (latest payslips/pension card/child support grant card)
- Sworn affidavit by parents/guardian if unemployed
- Certified copy of death certificates in the event that mother and/or father are deceased
- Proof of address (utility bill or letter from Councilor)
- Certified copy of your landlord’s ID copy if renting private accommodation.

- Consent Form: Download, complete and sign the form
- SA Identity Document: Certified copy ID(not older than 3 months, Certified copies of parent/s or guardian ID or Death certificate
- Proof of Income: Only if the non-SASSA recipient
- PWD(Persons with Disability): Complete disability form (Annexure A)
