- Both parents/guardians’ Certified ID copies
- Both parents/Guardians’ proof of income (latest payslips/pension card/child support grant card)
- Affidavit by parents/guardian if unemployed
- Certified copy of death certificates if mother and/or father are deceased
- Proof of address (utility bill or letter from Councillor)
- Certified copy of your landlord’s ID if renting private accommodation.

- Consent Form: Download, complete, and sign the form
- SA Identity Document: Certified copy ID(not older than 3 months, Certified copies of parent/s or guardian ID, or Death certificate
- Proof of Income: Only if the non-SASSA recipient
- PWD(Persons with Disability): Complete disability form (Annexure A)
